From primary care offices to a broad range of specialists, HealthShare Exchange (HSX) offers services to assist in providing quality and timely information that benefits patients and providers.

Currently, there are more than 120 independent ambulatory practices utilizing a variety of services to increase efficiency in the office setting and improve patient satisfaction and outcomes.

Whether it’s being notified that a patient has just been discharged from a HSX-member hospital or receiving information about a newly referred patient prior to the patient’s visit, HSX can improve the value of each patient encounter that comes with the sharing of data at the right place and time.

Service Descriptions 
  1. Direct Secure Messaging (Direct) / Provider Directory (PD)
    • Point-to-point exchange of clinical information between providers using the national Direct Project standards — included, but not limited to, sending of continuity of care (C-CDA) documents with discharge information and referrals for transitions of care. HSX can offer direct addresses to providers and organizations as needed.
    • The HSX PD contains more than 10,000 direct addresses for healthcare providers who actively participate with HSX for direct messaging; interoperability testing is completed before providers’ direct addresses are added to the PD to ensure practices can engage with Direct in a meaningful way.
  2. Automated Care Team Finder (ACTF)
    • Leveraging the Provider Directory and Health Plan provider attribution, HSX uses its ACTF to route continuity of care (C-CDA) documents, containing discharge information, from participating HSX-member hospital to patients’ primary care providers.
    • Currently, more than 3,000 C-CDA documents are delivered each month.
  3. Encounter Notification Service (ENS)
    • ​Practices and Clinics subscribe to panels of patients and receive timely notifications when their patients are hospitalized or present to an emergency department and then are subsequently discharged.
    • Admission, discharge, transfer (ADT) feeds from more than 50 member facilities in the region support ENS today.

  4. Clinical Data Repository (CDR)
    • Collects and retains medical-records data about patients, permitting a patient’s provider and/or care-team members to query for clinical information.
    • Ambulatory practices and clinics have the opportunity to access the CDR through the HSX query portal or through integration with an electronic health record (EHR) for their patients.

  5. Clinical Activity History (CAH)
    • Collects recent clinical history about a patient from the patient’s healthcare insurer, based on the patient’s member identification within the health plan and translates this claims/utilization data to a clinical care document — returning this information to the requesting provider.
    • Admission, discharge, transfer (ADT) feeds are needed from practices to trigger the clinical activity history document delivery.

  6.  CPC+ Performance Reporting Portal (PRP)
    • Practices participating in the Greater Philadelphia Comprehensive Primary Care Plus (CPC+) Program will be able to access quarterly performance reports derived from claims data.
    • Practices have the ability to review measures within the reporting period, view individual and practice level data and monitor trends over time.

*Access to the Performance Reporting Portal (PRP) is restricted to practices and providers participating in the Greater Philadelphia CPC+ Program. Participating practices contracting at any level will be able to access the PRP.
