Under a grant from the Commonwealth of Pennsylvania’s Department of Human Resources, Health Information Exchanges across Pennsylvania are collaborating to develop PA Navigate – a web-based platform that connects residents with social services that can keep them healthy and engaged in their communities as they age.
This HSX University session will focus directly on the Community-Based Organizations (CBOs) – helping them better understand what PA Navigate is, how it securely connects CBOs with others in the healthcare ecosystem, the benefits it provides to CBOs, and how CBOs can get involved.
