HealthShare Exchange Announces 2023 HSX Champions

PHILADELPHIA, PENN. – March 13, 2024 – HealthShare Exchange (HSX), an industry leader in clinical data sharing to support care management, quality improvement and population health for the Pennsylvania, New Jersey and Delaware healthcare communities, today announced its 2023 HSX Champions. Each year, HSX honors a group of individuals and teams that have gone above and beyond to promote HSX within their organizations, as well as extend the value and capabilities of HSX to other organizations. 

The 2023 HSX Champions are:

  • Theresa Moran, RN, Director of Care Management at Spectrum Health Services – As a care management leader, Theresa Moran utilizes admission, discharge and transfer (ADT) data to support transition of care efforts for Spectrum patients. A self-proclaimed “HSX fan girl,” Moran asks insightful questions and is a trusted source for constructive feedback to improve how HSX supports and engages with its members. Moran is also a key stakeholder in Spectrum’s participation in PA Navigate as the first federally qualified health center (FQHC) to integrate into the platform.
  • Jane Stone, Project Management Specialist, Information Technology at Grand View Health – Jane Stone has been instrumental in the development of the partnership between Grand View Health and HSX. She is assisting with various data efforts and expansions, including adding an additional HSX data set to support the organization’s ambulatory practices. Stone is also ensuring that the document query and retrieval service to access HSX clinical documents is optimized for Grand View Health’s providers and users. 
  • Bill Maroon, Chief Operating Officer; Kristina Scalia-Jackson, Director of Center of Excellence; Rana Gardner, Lead Services Navigator; Haneef Hill, EHR Analyst, Pathways to Housing PA – The Pathways to Housing PA team’s work with HSX is demonstrative of the effort to address social determinants of health (SDoH). The team leverages HSX’s Encounter Notification Service to identify unhoused individuals in the Philadelphia region and connect them with healthcare, mental health, housing resources and more. HSX’s data will be vital to their efforts to end homelessness in Philadelphia and their goal to provide housing for additional individuals this year.
  • Dr. Doron Schneider, MD, MBA, FACP, Vice President, Population Health and Clinical Strategy at Tandigm Health – As a population health company, Tandigm is implementing real-time information systems for clinical data and Encounter Notification to help their Complex Care Team and their network of affiliated primary care providers, specialists and health systems provide better care for patients across the continuum. Dr. Schneider is a leader in the company’s efforts in the design and implementation of population health management tactics, many of which rely on data exchange, to directly impact the health of patients as well as the wellbeing of care teams.
