Yolande Greene, Senior Project Manager for Population Health at HSX, and Nathan Hecker, HSX’s Technical Operations Lead, spoke on “Health Information Exchange (HIE) for Home Health: A micro and macro view.” A diverse and attentive group of Bayada staff, including corporate office leaders, care coordinators, and members of the research analytics team took in the presentation.
With the speaker series, organized by its Research, Analytics & Innovation Office, BAYADA seeks to promote its company’s creativity, problem solving, and care improvements, as it seeks to advance its role as a home health provider. In particular, care coordination and population health management are becoming increasingly important, amidst the increasing emphasis in the healthcare system on value-based care. The HSX team described health information exchange and its ability to support these areas.
At Bayada headquarters is in Moorestown Township, New Jersey, Yolande and Nate reviewed key services of HSX, including the Encounter Notification Service, Clinical Data Repository (CDR) Service, and Direct Secure Messaging. They explained how these health information exchange tools can strengthen the following areas:
- population health program evaluation
- quality improvement
- performance management
- disease or health risk monitoring and public health reporting
- outbreak investigations (exclusive of health department investigations)
- community health needs assessments
- and prevention services.
They outlined the assets that access to HSX data brings:
- historical data extraction
- HSX-generated reports and custom reports
- participant-subscribed reports
- dashboards (Live Data)
- and real-time data access for public health authorities.
