For ENS, the participating HSX hospital/health system members send notifications into the system when a patient is admitted, discharged, or transferred. As of June, for example, Abington Health, Aria Health, Crozer Keystone Health System, Doylestown Health, Einstein Healthcare Network, and Main Line Health were providing these data feeds to the service. Through its ENS vendor, HSX coalesces the alerts and, in turn, relays them to other providers within the system to inform care. During the spring, HSX began routing these notifications to practices/care managers caring for a high census of patients from HSX healthcare insurer member Health Plans Partners. In this case, Delaware Valley Community Health began receiving the notifications in May, and Temple University Physicians and Temple Physicians Inc. began receiving these encounter notifications in June, so that clinicians in these clinics and practices know when one of their patients has a hospital encounter or transition. HSX has also activated the service for patients of Independence Blue Cross and AmeriHealth Caritas.
In the Clinical Activity History use case, a participating hospital linked through HSX to an insurer can use the member ID of an admitted patient to request a recent medical history on the patient. The service commenced this July, when Main Line Health’s Paoli Hospital was enabled to request this information from Independence Blue Cross. Upon receiving the request, IBC translates claims information into a recent clinical activity history document listing recent procedures, medication list, and primary physician among other information, and sends the document via Direct secure messaging back to the hospital. With this information in hand, clinicians provide patients with more-informed care, delivering the correct treatment faster and avoiding repeat testing.
HSX expects use of these services to expand quickly among its members in the second half of 2015.
